  1. FILL OUT and then PRINT this form and send in with your check or money order made payable to F.S.T.S LLC.

  2. Send your registration form and payment to F.S.T.S. LLC, PO BOX 2885, Tulsa OK 74101-2885

  3. Within 7 -14 business days you will receive a receipt for your payment, a practice test, a workbook in which you will need to study ahead of time, and an enrollment conformation letter.

Indicate class or test and date requested by placing a check mark in the box next to your selection.

Note: We require one registration form for each individual enrolled. Thank you.
